She has feet somewhere under the skirts, but very often flies. That skirt was quite a problem at lower levels, but as her magic got stronger it was more easily handled. She made herself a pair of Garters of Levitation as a wondrous item.

The point with a small head is that it makes the whole figure look smaller, as smaller creatures/people have proportionally larger heads. Of course, this might make it look impish. All a matter of opinions.
